What’s Covered in This Agreement

This agreement includes:

  • Your understanding and agreement to participate in sessions

  • Confidentiality and its limits (e.g. legal obligations)

  • Fee structure and cancellation policy

  • Consent to engage in creative or educational experiences

  • Optional permission for documentation (e.g. artwork photos, therapist notes)

Consent is not just a signature — it’s an ongoing conversation.
You're always encouraged to ask questions, seek clarity, or change your mind about any part of the process.

We invite you to take a moment before you begin. This form is a space to share what feels right for you.

You’re welcome to respond in as much or as little detail as feels comfortable — there’s no pressure, only an open door. Every story begins somewhere. We’ll meet you right where you are.

Art Therapy Consent & Studio Agreement

REL Artizan Studio


Scope of Service

REL Artizan Studio provides non-diagnostic, arts-based therapeutic support for emotional wellbeing and creative expression. Art therapy is not a replacement for clinical psychology or psychiatric care. If more specialised or medical care is required, appropriate referrals may be recommended.​


Confidentiality

Sessions are confidential. Your personal information, creative work, and session notes are treated with care and protected by Australian privacy law. Information may only be shared:​

  • With your written consent (e.g., to collaborate with another professional)

  • If there is a risk of harm to yourself or someone else

  • If required by law (e.g., subpoena or court order)​

Session records are securely stored and retained for 7 years, or for minors, until age 25 - whichever is longer.​


Informed Consent

Art therapy is a voluntary and creative process. You (or your child) will never be pressured to talk or create. Participation happens at a pace that feels comfortable and self-directed.​

By signing this agreement, I acknowledge that:

  • Art therapy is not diagnostic or clinical

  • Participation is voluntary

  • Outcomes vary and are individual

  • The therapist is not liable for decisions made outside of session time

  • I can withdraw or pause participation at any time​


Session Fees & Cancellation Policy

Session Fee: Creative Healing Sessions are $150 per 90-minute session, inclusive of therapeutic support and selected studio materials.​

Studio Materials Fee (if applicable): An additional studio fee may apply, typically starting at $35, to cover specialised materials, framing, or extended mixed media processes. This will always be discussed ahead of time and clearly reflected in your invoice.​

Payment Methods: Payment is due on the day of the session via EFT, Square, or bank transfer, unless arranged otherwise (e.g., NDIS self/plan-managed).​

Cancellation Policy:

  • Cancellations made with less than 24 hours' notice may incur the full session fee

  • Rescheduling may be offered when possible

  • Emergencies and compassionate circumstances will always be considered with care​
