Creative Materials and Consent Statement

REL Artizan Studio celebrates creativity and expression. We sometimes capture or collect creative materials such as photographs, video, audio, written reflections, voice recordings, digital artworks, artworks-in-progress, or other creative content for educational, reflective, or promotional purposes.

Please read this form carefully and indicate your consent for how images, recordings, or creative materials of yourself or your child may be used. Participation is voluntary, and consent may be changed or withdrawn at any time by contacting the studio in writing at rel@live.com.au or via the Contact page on our website.

Creative materials and recordings may be used by REL Artizan Studio for related educational, therapeutic, reflective, and promotional purposes. These may include, but are not limited to, internal or external marketing, exhibitions, website and social-media content, research or publication, and educational or creative projects.

Materials (including images, audio, video, or written words) may be adapted or repurposed only within the scope of these purposes and will never be altered in a way that misrepresents the creator’s original intent. REL Artizan Studio will not use any material in an inappropriate or misleading manner. Identifiable information will never be shared without consent.

Once published, REL Artizan Studio has no control over materials shared in the public domain (for example, on social media).

This consent remains valid until withdrawn in writing. You may revoke consent at any time by contacting the studio in writing at rel@live.com.au.

Where the participant is under 18 years of age, this form must be completed by a parent or legal guardian.

All personal and creative information will be managed in accordance with REL Artizan Studio’s Privacy Policy, the Australian Privacy Principles, and the NDIS Practice Standards.

Confidentiality and Intellectual Property Notice

All information collected through this form is handled in accordance with the Australian Privacy Principles, the NDIS Practice Standards, and REL Artizan Studio’s Privacy Policy. Creative and photographic materials remain confidential and are used only for the purposes described above. All original artworks, therapeutic frameworks, and studio programs remain the intellectual property of Robyn Laycock and REL Artizan Studio.
